Please Notice:

  • Our counselors will provide you with: Requisition number - Confidential Code number - Login Password
  • Testing must be Prepaid
  • You must have your Requisition Number faxed to the Service Center before you have your sample taken.
  • Walk in the Service Center are provided only for Specimen Collection.
  • The Center is staffed by Certified Phlebotomists and Lab Technicians.
  • An additional Sample draw fee of $9 is applied to the cost of the STD Tests.
